Would love to have your suggestion to update the Chart sheet layout. It’s dated.

I have scanned it a few times but get the terrible scanned result that it’s not only twisted but also cut off some text, the last two lines.

But we’re really looking at the layout, not the text anyway. So please click the above hotlink to have a look and give your comment as how to improve the layout.

    I don’t think a logo is essential, but it will make the chart seem more official. The only other suggestion I would make would be to update the appearance of the chart. Try a different font and more modern grammar/verbiage.

    I would have something to brand you there it also frames the context of the form (ie you need to have the fact your THE EXPERT…as this frames the thinking when moving down the chart)

    Just make it simple and easy to follow…eliminate anything that maybe hard to understand for the lay person

    I second Steve’s comment. You want to establish VALUE by using YOUR logo. Use Last Name rather than Surname.

    I would create more space and room. Have it be more of a REPORT than a 1 page chart or something like to further establish the HUGE VALUE.

  7. John – I would certainly redo this chart and get your logo on their. Using an “industry standard form” could give your work a commoditized feel. Making it your form will greatly enhance your stature.

    I think you could make something yourself that looks WAY more fun than this!

    Put your name, your logo in there.

    Give the document more space, use more familiar language.

    I don’t really understand the Karmic stuff – so I might feel a little intimidated if I was asked to fill this out. Maybe cut out the stuff that won’t be obvious to everyone.

    I don’t think a logo is necessary but right now it is pretty boring. I would re-create and use a different font and layout.

    I would make each section more distinct by putting a box around them, and I would put a larger font Headline on each section, probably make it two pages, and make the entry areas much larger. I would also put a light gray (very light) on all areas where there is going to be information added by the user.

    Make it look easier to fill out.

  11. It needs to be more interesting but not in a trendy or gimmicky kind of way.
    Use a logo, it may not be necessary but will act like a signature.

    I think the layout is fine. Dressing it up via a logo wouldn’t hurt, but it looks fine to me. If I were a client of yours, and you demonstrated that you were highly knowledgeable in the topic, and you gave me the form, I wouldn’t think anything of it. It’s much similar to the forms that doctors give patients on their first visit to the doctor’s office. Those forms are generally just text with no frills.

    It appears that you will have your client fill out the very top portion (name, birth date) and that you will fill out the remainder of the sheet. Or will the client fill out the entire form?

    Customer does not need to fill out the form at all. I do it & give it to them as reference when they listen to their recorded reading.
